Legal Notice REQUEST FOR PROPOSALS The Calhoun County Board of County Commissioners is seeking professional consultant services for Design Services on the C.R. 69A paved shoulder project. The scope of this project will include the surveying and design of paved shoulders on C.R. 69A, including pavement markings and signage. The limits of the project are from S.R. 69 to C.R. 194 (Macedonia Road). FDOT FPID No. 429852-1-38-01 and 429852-2-38-01. CONSULTANT ELIGIBILITY: It is a basic intent of the County’s contracting program that contracts are procured in a fair, open, and competitive manner. By submitting a Letter of Response, the Consultants certifies that they are in compliance with FDOT Procedure No. 375-030-006, Restriction on Consultant’s Eligibility to Compete for Department Contracts. This directive is available on FDOT’s Web Site. FEDERAL DEBARMENT: This project is federally funded with assistance from the Florida Department of Transportation and the Federal Highway Administration. By submitting a Letter of Response, the consultant certifies that no principal (which includes officers, directors, or executives) is presently suspended, proposed for debarment, declared ineligible or voluntarily excluded from participation on this transaction by any Federal Department or Agency. SELECTION PROCEDURE: This project is covered by the selection process detailed in the Consultants’ Competitive Negotiation Act, Section 287.055 of the Florida Statutes. Selection will be made directly from Letters of Response for this project. After ranking of the consultants, the contract fee will be negotiated in accordance with Section 287.055, Florida Statutes. Note: The final selection date and time is provided in this advertisement. All public meetings will be held at the Calhoun County Board of Commissioners’ meeting room, 20816 Central Ave. East, Blountstown, FL 32424. In order to ensure a fair, competitive, and open process, once a project is advertised for Letter of Response or Letters of Qualification, all communications between interested firms and the County must be directed to Mr. Don A. RESPONSE PROCEDURE: Qualified consultants are encouraged to submit the original and three (3) copies of the letter of response to the Calhoun County Clerk’s Office, Calhoun County Court House, 20859 Central Ave East, Room 130, Blountstown, FL 32424, by the response deadline, Monday January 9th, 2012 at 12:00 p.m., C.T. The consultants will be ranked by the scoring committee. Selection will be made directly from Letters of Response by the Calhoun County Board of County Commissioners at 5:00 p.m., C.T. on Tuesday, January 17th, 2012. Letters of Response are limited to five pages for this project. Letters of Response should at a minimum, include the following information: a. Project Name/DOT Financial Management Number. b. Consultant’s name and address. c. Proposed responsible office for consultant and hours of operation. Office must be open five days per week from 7:00-4:00 CT and staffed with licensed professional engineer, surveyor and drafter knowledgeable with AutoCAD software. d. Contact person, phone number and Internet email address. e. Statement regarding previous experience of consultant or sub-consultants in advertised type of work. f. Proposed key personnel and their proposed roles (do not include resumes) g. Sub-consultant(s) that may be used for the project. h. Indication as to whether the prime firm and/or sub-consultants are disadvantages business enterprises (DBE). i.
