Hall, Interim City Manager Legal Notice Request for Letters of Interest and Qualifications The Apalachee Regional Planning Council is seeking qualifications from entities interested in coordinating transportation services for the transportation disadvantaged in Calhoun County, Florida. The selected entity will be the designated Community Transportation Coordinator (CTC) for the Transportation Disadvantaged Program, as authorized by Chapter 427, Florida Statutes (F.S.) and more fully described in Rule 41-2, Florida Administrative Code (F.A.C.). The CTC is defined by Chapter 427, F.S., as a transportation entity recommended by the appropriate designated official planning agency to ensure that coordinated transportation services are provided to the transportation disadvantaged population in a designated service area. The CTC has full responsibility for the delivery of transportation services for the transportation disadvantaged as outlined in s. 427.015(2), F.S. Interested entities are required to provide the following as proof of qualifications: description of organization, an organizational chart, capabilities and background information, prior work accomplishments, explanation of coordination experience, scheduling and routing software used by the entity, a list of vehicles to be used (if applicable); proof of insurability, credit references, a current financial statement and a current Medicaid provider number. Interested entities should submit four (4) copies of their expression of interest and qualifications in a sealed envelope to the Apalachee Regional Planning Council, 20776 Central Avenue East, Suite 1, Blountstown, Florida 32424 by 12:00 p.m. Central Time on Friday, August 5, 2011. The envelope must be marked, “LETTER OF INTEREST AND QUALIFICATIONS FOR CALHOUN COUNTY CTC.” Faxed and emailed responses WILL NOT be accepted.
